The form I Attach, Its admission Form for MLT Course at NUMS 2016-17 Admissions, (Last Year July 2016 Intake)
(Y) This form is called as Form C, likely there was 3 more Forms last year on NUMS Website, 
(Y) Form A.
(Y) Form A1.
(Y) Form B.
(Y) Form C.
(Y) Form D.
(Y) #Form_A was for MBBS,BDS Admissions for NET 1, (NUMS 1st Test ,held on 7 August 2016).
Form A1 was for MBBS,BDS admissions for NET 2,(NUMS 2nd Test,Held on 25 September 2016)
Form B was for Cardiac perfusion Course and for BS Nursing (Y) (Note, This Nursing is as a Civilian, not AFNS Cadet, for Cadet you should have to apply at ASRC,when AFNS Registration will be open,likely in June/July)
Form C was for BS Medical laboratory Technology Course
Form D was for PhD Courses.
..
So first u must be aware of these all forms,which form is for which course, u can apply for only one course at a time, except MBBS/BDS, and Cardiac Perfusion / Nursing for which there is a same form Form A/A1 and Form B respectively, 
You can tick both the courses to apply for both MBBS / BDS or CP/Nursing, while for MLT You have to download other form,which is Form C, 
(Y) #Note, You can't Apply for Both, MLT and MBBS/BDs, or Cardiac Perfusion and MBBS/BDS,
Those Students applying for MLT can't apply for any other course.
(Y) 1st u must know the Form Category for Respective Course, 
(Y) 2nd You will have to download respective Form for which course you are going to apply.
(Y) 3rd Fill that form, must read the instruction given at the second page of Form, (Form will be in PDF File,You will have to print it out, then fill that form
(Y) 4th, Then Attach the required Documents,(2 Photograph,one attested from back, other front attested, and Your CNIC/B.Form)
(Y) 5th. Then you should have to pay bank chalan, This Chalan is The fee for that test,.
Bank Challan Amount 
1500 PKR for MLT/CP/Nursing
3000 PKR MBBS/BDS/M.Phil/Ph.D 
 (Paid bank Chalan will also be attach with that Admission Form),
Note: Bank Chalan will be at the 3rd page of NUMS Admission Form.
Then you will have to send that properly Filled Admission Form to the NTS Address, NTS address will be given at the 2nd page of NUMS Admission Form.

(Y) Then you will have to wait till your Roll No.Slips generated by NTS, before 1 week of your test date.
Note. Roll No. Slips will be sent to you either by TCS, or by E.Mail address u provided, or both, If in any case you didn't receive neither mail nor TCS then you should have to visit NTS Website, your roll no slipp will be there, download it, or take Screen Shot of your slip from website, and print it out, 
On The Test Day, You will have to bring that Slip with you, otherwise no entry in Exam hal.

after the test will be over Answer key will be uploaded at nts website with in 48 hours.
Official Result of MBBS/BDS applying students will be announced after 2nd test.while MLT/CP/Nursing applying students Result will be announced with in 15 Days.

(Y) #Note: There will be only 1 test for MLT/CP/Nursing Course, while for MBBS/BDS course there will be 2nd test held in september, You can apply either for NET 1, or NET 2 or both, Best Score will be considered for Merit List.

after the result announce for MLT/CP/Nursing Course, for those students there is another Admission form will be uploaded at NUMS Official website, you should have to download that form, Fill that Form properly, attach the required Documents with that form, there will be no extra fee for that form, and send that form before the mentioned date on the form to the NUMS address.(NUMS address will be mentioned on that form at the 2nd page)

(Y) Final merit list will be made by NUMS Directorate , and Selected Students will be informed by E.Mail, and Call Letters too.
Those selected candidates will have to Report at NUMS on the date given by NUMS, for Further documentation and Dues submission, and Finally they will have to report at Armed Forces Post Graduate Medical Institute CMH Rawalpindi.

while on the other hand after the 2nd test of MBBS/BDS will be over, Official Result of both the test will announce , then another admission form for Admission in Army Medical college MBBS/BDS will be uploaded at NUMS Official Site, you will have to download it, print it out, fill it carefully, attach the required documents, also medical fittness certificate will be attached with that form, you will have to examine your nearest Doctor, He will declerify you medically fitt.then you will have to send that form to the NUMS address mentioned on the 2nd page of that form.

(Y) #Note: You will enter your nums test result for which you scored more, you will not enter both the result, only that result in which u scored more,.

after that nums will made the merit list of all the students who applied for Army Medical College

#Major_mistake done in the last year:
Most of the students didn't apply for ARMY Medical College after NUMS Result,.They thought we had done our test, NUMS will automatically make the merit list now,That was the major mistake done by thousands of students last year in 2016, so keep in mind you have to apply for AMC, after the nums result announcement then your name will be in that merit list, other wise you will never found your name in that NUMS merit list.

At the end 1st merit list will be displayed at nums wesbite, and selected cadets will be informed by E.Mail and call letters too.

1st merit list will be displayed in November, selected cadets will have to report at AMC on given date for further Documentation and dues submission,. 
Note: 1st semester AMC Dues will be 136700 PKR.

2nd merit list will be announced , if any of thg selected students will not report at AMC, in December, 3rd List will not be displayed, rather only selected students for 3rd list will be individually informed to report at AMC.

This was the whole procedure For applying as NC (Nums Cadets, Civilian) 
while for PC (Paying Cadets) after Nums test result announce you will have to apply direct getting PC Forms from GHQ,AHG,NHQ, for Army,PAF,Navy reserved Seats Respectively .
Your Merit will be made by GHQ,AHQ,NHQ, 
On Your selection you will be informed by GHQ,AHQ,NHQ by cal letter,
and you will have to report at AMC on the given day ,..
